1. Health Issue (1 solid paragraph)
Describe the major health problem that you are focusing on and the target population. Do not
confuse health problems with health behaviors. A health problem is a condition, e.g., obesity or cancer. Health behaviors are poor diet, smoking, and heavy drinking. In 1-2 sentences explain why you are proposing this project (e.g., high prevalence rates in the target population and lack of interventions in the community to address the problem).

2. Public Health Interventions (1-2 pages)
Describe two empirical studies on interventions designed to address your health problem.
Explain which type of health promotion strategy was used, e.g., health education, health
communication (e.g., mass media), policy, individual counseling, etc.
What were the main components and activities included in the intervention? Be specific
about program scope, multiplicity, and duration of the intervention.
What was the target population (e.g., gender, race/ethnicity, age, location)?
Identify the short-term impacts and/or long-term outcomes that were measured in the study?
Describe the measures used to assess outcomes
Describe how the intervention your team is planning will adopt or adapt one of the
interventions you found in the research literature.
